Adult Programs & Services
Dental
Dental Services – including exams, cleanings, sealants and fillings – are provided to uninsured children and those with TennCare, under the age of 21.  Adults with no dental insurance coverage are only seen in cases of emergency.  Fees for services are on a sliding fee scale  based on family size and income.

If you or your child is a new patient or have not updated your health history in more than one year, please print and complete the Dental Health History questionnaire (
EnglishSpanish)  and bring to your appointment along with a list of any medications you or your child is taking.

Family Planning
The family planning program provides physical exams, cervical cancer screening, breast and testicular cancer screening, testing and treatment for sexually transmitted diseases, and pregnancy testing. Women and men are educated about various birth control methods, and supplied with their chosen method (unless there is a medical reason that would be dangerous).

If you are a new patient, please print and complete the Health History questionnaire (
EnglishSpanish) and bring to your appointment with a list of any medications you are taking.

Immunization "Shots"
Immunizations against polio, diphtheria, whooping cough, lockjaw, measles, mumps, rubella (German measles), Hemophilus, and Hepatitis B are provided for children. Influenza (flu) and pneumonia immunizations and diphtheria-tetanus boosters are also available to adults for lasting protection against these diseases.

Prenatal
Pregnancy testing, prenatal vitamins, referrals and temporary enrollment on TennCare are available. Please ask us which services we offer.

Primary Care
Medical staff is available to provide routine preventive care, sick care, and routine physicals for diagnosing and treating acute and chronic illnesses. Preventive screenings such as pap smears, colorectal, prostate and other tests can be conducted to screen for cancer. Referrals services are also available.
The health department participates in TennCare, and accepts Blue Cross Blue Shield Network S, P and E insurance. Fees for services are on a sliding fee scale based on family size and income.

If you are a new patient, please print and complete the Health History questionnaire (EnglishSpanish) and bring to your appointment with a list of any medications you are taking.
