REPORT FRAUD, WASTE AND ABUSE

ALLEGATION:

Please provide as much detailed information as possible concerning the event that you believe constitutes fraud, waste, or abuse. It is imperative that you provide complete and accurate information to assist the Independent Audit Committee in reviewing the incident.

WHAT:

Provide a detailed description of the alleged fraud, waste, abuse or mismanagement. Please include the following information if known: (1) the full name of the person or persons involved in the misconduct and the office where the person(s) works; (2) a description of the misconduct; (3) how you know about the alleged misconduct; (3) how and when the misconduct occurred; (4) when was the misconduct discovered; (5) the amount of money involved; (6) how long the alleged misconduct lasted; and (7) any other information you believe may be relevant.
